I namely Sulaiman Altaf Kapadia (with Indian passport number K4805446) father of a minor son Mohammed Sulaiman Kapadia aged 7 years with Indian passport bearing the number T3916249 issued at Jeddah do declare that I am changing the name of my Son from Mohammed Sulaiman Kapadia to Rayyan Sulaiman Kapadia. The new name shall come into effect from today for all the purposes.
